IMPORTANT PATIENT INFORMATION WITH IMMEDIATE EFFECT:

⚠️Online access to appointments has been switched off.

⚠️ALL patients phoning the surgery will be asked about COVID-19 risk before an appointment is booked.

⚠️ALL patients will be asked if a clinician can ring them rather than attending the surgery.

⚠️On call triage clinicians will telephone triage any patient with respiratory symptoms and refer them to NHS 111 as necessary.

DO NOT ATTEND THE SURGERY IF YOU ARE EXPERIENCING A TEMPERATURE OF 37.8C OR A NEW CONTINUOUS COUGH. SELF ISOLATE AT HOME FOR 7 DAYS AND CALL 111 IF SYMPTOMS WORSEN.

We are grateful to our patients for working with us to reduce the footfall in the surgery as much as possible, thus minimising infection risk to themselves and others.

 REPEAT PRESCRIPTIONS

As a measure intended to protect you and others  from the potential risk of infection, PLEASE do not come to our reception to order medication.  To reduce footfall in our surgery please:

⚠️ Register for on-line patient access

⚠️ nominate a pharmacy for electronic prescribing if you haven’t already done so;

⚠️ask your pharmacy to order your medication for you;

⚠️ write to us with your request; 

⚠️email us monksparksurgery@nhs.net

ONLY AS A VERY LAST RESORT SHOULD YOU DROP A PRESCRIPTION REQUEST IN TO THE SURGERY PERSON

Lastly, if you have regular medication please don’t request too early as your request will be rejected. Please continue to use the service responsibly as you usually would.
